目的:探究胆道镜联合钬激光在肝内外胆管结石治疗中的应用价值。方法:选择2014年6月至2015年6月茂名市电白区人民医院收治的68例肝内外胆管结石患者作为研究对象,根据治疗方式的不同分为单独组和联合组,每组各34例。单独组实施胆道镜手术,联合组采用胆道镜联合钬激光手术。观察两组患者取石成功次数及术后并发症发生情况。结果:联合组首次取净结石率为97.06%,优于单独组的79.41%(P<0.05);联合组术后并发症发生率为2.94%,低于单独组的17.65%(P<0.05)。结论:胆道镜联合钬激光手术治疗肝内外胆管结石可取得较为显著的治疗效果,有效降低术后并发症发生率,值得临床推广。

Objective:To investigate the use of choledochoscopy and holmium laser surgery in the treatment of intrahepatic and extrahepatic bile duct stones. Methods:Sixty-eight patients with intrahepatic and extrahepatic bile duct stones,who were hospitalized in the People's Hospital of Dianbai District in Maoming between June2014 and June June 2015,were included as the subjects in the study. According to the different treatment methods,all patients were divided into the single treatment group and combined treatment group( n = 34 each).The single treatment group received choledochoscopy,and the combined treatment group received choledochoscopy and holmium laser surgery. The number of successful stone removal and the incidence of postoperative complications were determined. Results:The stone-free rate at the first time in the combined treatment group was97. 06%,which was higher than 79. 41% in the single treatment group( P < 0. 05). The postoperative complication rate in the combined treatment group was 2. 94%,which was lower than 17. 65% in the single treatment group. Conclusion: Choledochoscopy and holmium laser surgery in the treatment of intrahepatic and extrahepatic bile duct stones can achieve a significant therapeutic effect,reduce the incidence of postoperative complications,and justifies widespread use in clinical settings.
