二维灰阶超声联合SMI在他汀类药物治疗颈动脉斑块疗效评估中的应用

摘要:

目的:研究二维灰阶超声联合超微血流成像技术(SMI)在他汀类药物治疗颈动脉斑块疗效评估中的应用效果。方法:选取2018.1至2019.4我院门诊、心血管内科、神经内科录入符合入选标准的70例患者为研究对象,均行二维灰阶超声联合SMI技术检测,观察斑块大小、斑块回声及斑块内新生血管情况,给予他汀类药物治疗,比较用药前及用药24周、48周后的二维灰阶超声联合SMI技术检测结果。结果:70例患者经过二维灰阶超声检查,共发现斑块102个,其中低回声斑块45个,以低回声为主的混合回声斑块57个。102个斑块经过SMI血流显像技术检查,筛选出有血流信号的低回声及低回声为主的混合回声斑块共计33个,斑块内血流显示率为32.35%(33/102)。用药24周,患者颈动脉低回声斑块及混合回声斑块的平均厚度及平均长度与用药前相比差异无统计学意义(P>0.05);用药48周后,患者颈动脉低回声斑块及混合回声斑块的平均厚度及平均长度均显著低于用药前及用药24周后,差异具有统计学意义(P<0.05)。结论:二维灰阶超声联合SMI能够有效评价他汀类药物治疗颈动脉斑块疗效,应用效果良好。

Abstract:

Objective:To study the application effects of two-dimensional gray scale ultrasound combined with superb micro-vascular imaging(SMI)in the evaluation of efficacy of statins on carotid plaques.Methods:From January 2018 to April 2019,70 patients enrolled in the Outpatient Department,Department of Cardiology and Department of Neurology of our hospital who met the inclusion criteria were selected as the research subjects.All patients were examined by two-dimensional gray scale ultrasound combined with SMI technology to observe the plaque size,plaque echo and plaque neovascularization.Statins were given to patients,and the examination results of two-dimensional gray scale ultrasound combined with SMI were compared before medication and after 24 and 48 weeks of medication.Results:Two-dimensional gray scale ultrasound examination for 70 patients found a total of 102 plaques,including 45 low-echo plaques and 57 mixed-echo plaques(mainly low-echo). SMI blood flow imaging technology for 102 plaques showed a total of 33 low-echo or mixed echo plaques(mainly low-echo)with blood flow signals were screened out.The blood flow display rate in plaque was 32.35%(33/102).After 24 weeks of medication,the average thickness and average length of internal carotid artery low-echo plaques and mixed-echo plaques were not significantly different from those before medication(P>0.05).After 48 weeks of medication,the average thickness and average length of internal carotid artery low-echo plaques and mixed-echo plaques were significantly lower than those those before medication and those after 24 weeks of medication( P< 0. 05).Conclusion:Two-dimensional gray scale ultrasound combined with SMI can effectively evaluate the efficacy of statins on carotid plaques,with good application effects.
