机器人早期辅助步行训练对脑卒中后偏瘫患者下肢运动功能的影响

摘要:

目的:探讨早期应用机器人辅助步行训练对脑卒中后偏瘫患者下肢运动功能的影响。方法:选择2016年6月至2017年8月惠州市中心人民医院收治的120例脑卒中偏瘫患者为研究对象,按照随机分配法分为对照组与观察组,每组各60例。对照组给予常规康复治疗,观察组给予早期联合应用机器人辅助步行训练。观察两组患者下肢功能的变化。结果:治疗前,两组患者各项步态参数、运动功能分级人数、独立步行人数比较,差异均无统计学意义(均P>0.05);治疗后,观察组步长、步宽、步频、步速等指标较治疗前及同期对照组有明显改善(均P<0.05);观察组下肢运动功能≥Ⅳ级者、独立步行人数均多于对照组(均P<0.05)。结论:早期应用机器人辅助步行训练有助于提高脑卒中后偏瘫患者的下肢运动功能,对于提高其生活质量,早日回归社会具有重要意义,值得推广。

Abstract:

Objective:To investigate the effect of early robot-assisted walking training on motor function of lower extremities in patients with post-stroke hemiplegia.Methods:A total of 120 patients with post-stroke hemiplegia admitted to Huizhou Central People's Hospital between June 2016 and August 2017 were included as the subjects in the study,and were randomly divided into the control group and study group(n=60 each).The control group received routine rehabilitation.In addition,the study group received the robot-assisted walking training at the early stages.The effect on the function of lower extremities was assessed.Results:There were no statistically significant differences in the gait parameters,number of patients by motor function gradings,and number of independent walkers between the two groups at the baseline(all P>0.05).After the treatment,the stride length,stride width,stride frequency,and stride speed in the study group were significantly improved compared with those in the control group(all P<0.05).The number of patients with motor function ≥ IV and the number of independent walkers in the study group were more than those in the control group(all P<0.05).Conclusion:Early use of robot-assisted walking training favors the improvement of motor function of lower extremities in patients with post-stroke hemiplegia,which is of great significance for improving their quality of life and early return to society,and justifies widespread use.
