对１２２例不同类型的急性脑血管病患者血清Ｄ－二聚体（Ｄ－Ｄｉｍｅｒ）检测，研究发现：①在急性缺血性和出血性脑血管病中，血清Ｄ－二聚体都有程度不同的阳性反应，其中脑血栓形成和脑出血阳性率较高（ ５７． ９％和 ５２． ８％，两者比较 Ｐ＞ ０． ０５），腔隙性脑梗塞阳性率较低（１４％，与前两者比较 Ｐ＜０．００５）。②Ｄ－二聚体阳性反应率与病灶的大小有明显关系，病灶体积＞ １０ｍｌ的阳性率（脑血栓形成为 ８４． ６％，脑出血为 ７８． ６％）明显高于体积＜１０ｍｌ（脑血栓形成成 ４４％，脑出血 ３６．４％）， Ｐ＜０．０５。提示 Ｄ－二聚体检测对急性脑血管病的诊断有一定临床参考价值，并对其产生机理进行分析讨论。

We examined D-dimer in 122 patients with acute cerebral vessel disease. The result suggested: In acute ischemic and bleeding cerebral vessel diseases, the serum D-dimer was positive in different degree, the positive reaction rate was relatively high in case of cerebral thlombosis and cerebral bleeding(57. 9 % and 52. 8 %, between them P >0. 05 ), and it was very low in lacunar cerebral infarction (14%, P < 0. 005, compared with the above two). There appeared to be significant correlation between D-dimer positive reaction rate and the mass of focus, the positive rate in focuses larger than 10ml was significantly higher than that smaller than 10ml (P <0. 05). So, it is concluded that the detection of D-dimer is valuable for the diagnosis of acute cerebral vessel disease. The mechanism for it is also discussed in the study.
